About

The SNAP (Supplemental Nutrition Assistance Program, formerly known as food stamps) program offers outreach and application assistance to determine eligibility for food stamps and medicaid and help with the enrollment process. Case Managers assist with the preparation and submission of SNAP applications and renewals to the Department of Children and Families. SNAP services are offered in Lee, Hendry and Glades Counties at various locations

ELIGIBILITY

The SNAP program offers application assistance to determine eligibility for food stamps.  Case Managers will assist in doing the application and submitting the entire application (along with required documents needed) to the Department of Children and Families.  SNAP services are offered in Lee, Hendry and Glades Counties at various locations throughout.

WHO IS ELIGIBLE (include location if applicable): Lee, Hendry and Glades counties

HOW TO ACCESS SERVICE/Intake: Call main phone number to set up an appointment at a location near you.

ITEMS NEEDED at intake (ie: id card, etc): U.S. issued Identification (birth certificate, drivers license, or identification), proof of income, and proof of residency (lease, mortgage, etc)

SUCCESS STORIES

Scott enrolled in our program and was identified as having special needs by Child Find. Staff have worked with Child Find to develop individualized tools and strategies to help him be more successful in the classroom. He has begun to make progress using sign language and picture cues to express his needs.
