WHO IS ELIGIBLE (include location if applicable): Students in K-8

HOW TO ACCESS SERVICE/Intake: Call or walk in to obtain registration form

ITEMS NEEDED at intake (ie: id card, etc): Parent/Guardian ID, Students BC, proof of income, recent report card, proof of address
